Erratum: Helicopter loss article in May/June 2017 Navigation News
Notice of erratum to article published Navigation News.
The article "E-Map Omission Costs Lives of Helicopter Crew” in the May/Jun edition of Nav News (p.6) included some factual errors that were not picked up in the pre-publication review process. These errors may appear to pre-empt the findings of the Irish Authority's accident investigation, which is still ongoing. The RIN apologises for these errors and for any distress or offence that they may have caused.
The article should have made clear that the lost aircraft was not itself going to pick up an injured seaman, but was deploying to provide top cover for another SAR helicopter which had taken off earlier to effect the rescue; that EGPWS equipment is a 'goalkeeper' warning device, and its displays are not intended to be used for navigation; and that other charts in the aircraft (possibly including some electronic charts) may have shown more detail of local terrain and obstructions.
A comprehensive Preliminary Report has found that controlled flight into terrain (CFIT) occurred, but the cause of the accident, and any contributing factors, remain to be established by the accident investigators.
